How do I request a Prescription Refill?

Click here to log into the patient portal and request a prescription refill.

Your request will be reviewed and processed during normal business hours. If you request a refill after hours or on weekends, it will be reviewed and processed on the next business day. We will need to have your chart available to determine that a refill is the best course of treatment. Please request renewals at least 72 hours in advance.

If you have not been seen by a physician at SDACC in the past 12 months, your request may be denied and you will be directed to request an appointment.

If you do not have a portal account, please call 937-433-8990 between 8:00 a.m. and 5:00 p.m. Monday thru Thursday and between 8:00a.m. and 3:45p.m. on Friday, so that we may have your chart available to determine that a refill is the best course of treatment. You should have the pharmacy name and phone number available. Please call for renewals at least 72 hours in advance.
