To register as a user for our patient portal, please call the office at (937) 433-8990. We will send you an invite to register for a portal account.

Please make sure to remember your user ID and Password for later sessions. For security purposes, the office cannot give out User IDs over the phone. If you have forgotten your password and remember your user ID you can click on the “forgot my password” button located on the log in screen.
