PATIENT PORTAL Instructions
Everyone to be seen needs to Create An
Account on the website AND complete a Patient
Registration Form the first time you visit the
Patient Portal.
To do this Click on Patients Portal

Click on
My Patient Page

Choose
Create An Account

Enter your
information into the required fields marked by
an *
o
Click
JOIN
·
Acute Care
Consultant (regular office practice) patients
that are NEW patients need to complete the
Personal Medical History form.
o
Established
office patients simply request an appointment or
other online service request.
o
Completed online
forms will be filed into your patient chart

If you are
coming here for Travel Consultation, select
either
new or established patient for Adventure and
Travel Medicine Center
at the bottom of Patient Pre-registration Form.
o
This will take
you to the ATMC Screening Form, which is
required when requesting a travel
appointment.
Once you have created an account or Logged in
and you are paying a bill online or requesting a
regular office appointment with Acute Care
Consultants, simply click on the link under
Quick Links or My Account and it will take you
to a screen to enter information for the task.
