 Welcome to South Dayton Acute Care Consultants’ (SDACC) website

 We appreciate the opportunity to assist you with your medical needs.  Our goal is to provide high-quality medical care and excellent service to our patients.

 Our website will allow you to do online registration, complete your medical history form and make payments on your account.

You can request prescription refills and review our financial policy.  You will be asked to sign the financial policy at the time of your visit.

  IMPORTANT VISIT INFORMATION:

•  Please bring your insurance cards.

• Please bring all medications or an up-to-date medication list.

• Be prepared to pay your co-pay at every visit.

          A charge will be made for late co-pays.

• A handicap entrance is located on the north side of our building off of the Far Hills entrance.

• Please check in with the patient care coordinator in Suite 101 at least 15- 20 minutes before your appointment time.

• Office hours are:

• Monday - Thursday 8:30 a.m. to 5:00 p.m.

• Friday 8:00 a.m. to 4:00 p.m.

•  APPOINTMENT Reminders

• We will make every effort to accommodate your appointment scheduling needs. 
• Please notify us at least 24hrs in advance if you are unable to keep your appointment. 
  • A charge will be made for ALL missed appointments unless 24hr notice is given.
