How do I get a user ID and Password for the website?

        To register as a user on our secure website please go to Patients Portal and click the "My Patient Page”. And go to create an account.  Once you have created a User ID and Password you can access any of our secure features.

Please make sure to remember your user ID and Password for later sessions. For security purposes the office cannot give out User IDs over the phone. If you have forgotten your password and remember your user ID you can click on the "forgot my password" button located on the log in screen.
