Prescription Renewals

 Please call 937-433-8990 between 8:00a.m. and 5:00 p.m. Monday thru Thursday and between 8:00a.m. and 4:00p.m. on Friday, so that we may have your chart available to determine that a refill is the best course of treatment. You should have the pharmacy name and phone number available. Please call for renewals at least 72 hours in advance. 

You may also request a Prescription renewal on-line.  Follow the directions as noted on this website.  All online requests will be processed on the next business day.  Please make online requests at least 72 hours in advance.

If you have not been seen by our physician at South Dayton Acute Care Consultants in the past 12 months, your request may be denied and you will be directed to request an appointment.
