How do I request a Prescription Renewal through the website?

If you have already created a User ID and password you can log in and request a prescription refill by Clicking here.

Your requests for prescriptions will be reviewed and processed during normal business hours. If you request a refill on-line after hours or on weekends it will be reviewed and processed on the next business day.  We will need to have your chart available to determine that a refill is the best course of treatment.  Please request renewals at least 72 hours in advance. 

If you have not been seen by a physician at South Dayton Acute Care Consultants in the past 12 months, your request may be denied and you will be directed to request an appointment.
