What is My Patient Page?

My Patient Page is your personal inbox.

Once you create a user ID and password you can access your inbox to securely communicate with the practice for the following:

·         Access all online services and forms offered by your physician

·   Complete forms

·         Request appointments

·   Request prescription renewals

·         Update demographic and insurance information

·         Update email address

·         Update user name & password

·         View history of previous encounters

·         Pay your bill online

You will need to choose a location while creating an account.

·         Regular office patients select Acute Care Consultants location. 

·         Travel patients select Adventure and Travel Medicine Center location.

This inbox is also where you will be directed to login when the office responds to your requests.
